Xcode iOS project only shows My Mac 64-bit but not simulator or device

I figured it out. I had to edit the scheme (Product->Scheme->Edit Scheme...), and for some reason no executable was selected. I chose my app, saved and now I have my simulator and device options back.

This happens to me every time files get modified outside of Xcode, and Xcode needs to reload the project file. Closing the project and re-opening it always fixes the issue for me.

Why can't I see any other simulators in Xcode?

It appears that you have configured this project as a Mac app. Therefore there are no "simulators", as there is nothing to simulate; the app runs directly on your Mac.

If you intended to configure this project as an iOS app, the best course is to throw this project away and start over, and this time, when you make a new project, be more careful about specifying what kind of project it is to be.

If you open the project in XCode (4.4.1), on the left top click the folder icon, then select your project icon to bring up the project settings page. Click on the "Summary" tab, then you should see "Devices" where you can select the device you want to target.

If that doesn't work (you don't see "iOS Application Target") you may need to go into "Build Settings" and select a different "Base SDK" -- I usually select "Latest iOS". You may have selected a Mac OS X SDK. Also check "Supported Platforms" to select "iOS".
